Copy of the front and back of your Texas Driver's License.
Copy of the front and back of your Social Security card.
Upload a copy of the front and back of your Blue Card to CastleBranch, then bring
the original Blue Card on the first day of New Student Orientation.
If you do not yet have your Blue Card, bring your receipt of the Texas Board of Nursing
Outcomes letter indicating you may sit for the National Council of Nursing Licensure
Examination for Registered Nurses (NCLEX-RN®).
2x2 passport-size photo* with a plain background and your name printed or typed on
the back of the picture.
Copy of the front and back of your current medical insurance card.
Copy of your background check results from CastleBranch.
Copy of your negative* drug screen report from CastleBranch.
*No diluted negative results accepted.
Copy of your CastleBranch summary report showing your current status as "completed" with your name at the top.
Copy of your immunization record and completed physical exam form to complete your clinical requirements.
Proof of your American Heart Association BLS (CPR) Certification.
Official transcripts showing completion of current prerequisite coursework.
To promote student success, you are required to attend an on-campus two-day nursing student orientation. Please bring your own lunch and dress in business casual attire.
Day 1: DATE TBA, 8 a.m. - 4 p.m.
The first orientation day will provide information about the application requirements,
program overview, expectations, and resources.
Day 2: DATE TBA, 12 p.m. - 4 p.m.
The second orientation day will focus on study tips, time management, test taking,
and test anxiety.
Location: C.11-1081 Health Science Building
If you are unable to participate in orientation, please notify your assigned San Jac ADN staff member before the start of the orientation.
Throughout your nursing education journey with San Jac, you will have your very own staff member available to assist you and ensure your success. You're assigned a specific contact based on your last name.
Central Campus
You should have submitted your name to the Texas Board of Nursing (TBON). They will
send you an outcome letter, known as a blue card. You'll need this blue card in order
to enroll and will need to bring it to nursing orientation.
The TBON will also send you information on the background check.
Those who receive a "Fast Pass" email from the TBON must complete the process described
in that email.
If you do not receive a "Fast Pass" email, don't worry. You will still receive what
you need from the TBON.
You're required to dress in uniform and have a lab pack on the first day of class. Uniform and lab packs must be purchased through TMC Scrubs N More. We encourage you to make an appointment as soon as possible with the following contact:
